Military Service Information: Branches of the United States Armed Forces in which you served and whether your service included Active Duty, National Guard, or Reserve components.

Administrative Information: General VA benefits process status, upcoming deadlines, referral sources, and screening acknowledgments.

Authorized Representative Information: If someone assists a veteran with the screening, we may collect that individual's contact information and the veteran's name. Additional authorization may be required before we discuss a veteran's confidential information with another person.

Our website provider may also process certain technical information, such as device, browser, or network information, to operate and secure the website.

3. Information We Do Not Request During Preliminary Screening

For your protection, please do not submit Social Security numbers, VA claim numbers, dates of birth, detailed diagnoses, complete medical histories, medical records, or other highly sensitive documents through the preliminary screening questionnaire.

If you are invited to proceed with a clinical evaluation, we will provide separate instructions for securely submitting the documentation needed for that service.

4. How We Use Your Information

Next Level Nexus uses preliminary screening information to evaluate whether our independent medical evaluation services may be appropriate for your needs.

This may include verifying the completeness of your submission, determining whether the requested evaluation falls within our available clinician's professional expertise and applicable licensing authority, and communicating with you regarding the screening decision.

We may also use limited information for internal administration, service quality assessment, operational planning, security, and compliance with applicable legal obligations.

Completing the questionnaire does not establish a clinician-patient relationship, create an agreement for paid services, guarantee a favorable medical opinion, or guarantee a particular Department of Veterans Affairs decision.

5. Who May Access Your Information

Access to preliminary screening information is restricted to authorized personnel and service providers with a legitimate business or clinical need.

Administrative personnel may review contact details, submission completeness, and information needed to coordinate services.

Authorized clinical professionals may review relevant screening information to determine whether a requested evaluation is appropriate.

Submitting a screening does not automatically authorize Next Level Nexus to share your information with the Department of Veterans Affairs or any veterans' organization.

These measures include restricting information access to authorized users, using individually assigned business accounts, protecting account access, and maintaining information within approved business systems.

7. Electronic Communications

By submitting the preliminary screening questionnaire and providing contact permission, you authorize Next Level Nexus to communicate with you regarding your screening inquiry.

We will endeavor to use your preferred communication method when practical.

Ordinary email and telephone communications may have privacy limitations. Please do not email medical records or detailed health information unless we have provided an approved secure communication method.

Permission to contact you regarding your screening is not permission to send unrelated promotional messages.

8. Information Retention

We retain preliminary screening information only for as long as reasonably necessary to administer screening requests, maintain appropriate business records, resolve inquiries, and meet applicable legal obligations.

Our retention and secure disposal procedures will be governed by our established records-management policies and applicable law.

When information is no longer required, we will take appropriate steps to securely delete or dispose of it.

You may choose not to complete the preliminary screening questionnaire. However, without sufficient information, Next Level Nexus may be unable to evaluate your inquiry.

You may contact us to request access to information you have submitted, request correction of inaccurate information, inquire about deletion, or raise a privacy concern.

We will evaluate these requests and respond in accordance with applicable laws, identity-verification procedures, and record-retention obligations.
